Liability Waiver

I, *[above entered name]*, residing at *[above entered address]*, do hereby waive as against the Northport Native Garden Initiative Inc., its officers, servants, agents, volunteers, or employees (collectively, “NNGI”) ANY AND ALL LIABILITIES, LOSSES, CLAIMS, DEMANDS, DAMAGES, COSTS, EXPENSES, JUDGMENTS OR INJURY, INCLUDING DEATH, that may in any way arise and be inure to and be sustained by me in connection with or as a result of my participation in any plant sale and related events organized and/or conducted by NNGI, including, without limitation, for injuries sustained as a result of the alleged negligence of NNGI.
In addition, by signing this I am giving permission to NNGI to record, videotape and photograph my image and use it in any NNGI promotional materials.
In signing this Waiver, I do hereby acknowledge and represent that I have read and understand the terms and provisions of this document, which I sign voluntarily as my own free act and deed. I execute this document for full, adequate and complete consideration fully intending to be bound by the same, now and in the future.
